4. Location of Presentation
5. Agreed upon Honorarium
6. Consultant’s Social Security Number
7. Down Payment by Purchase Order, agency warrant, or by Paypal Option
8. Due date of balance
9. AV Required and AV Room Layout
10. Books and Materials Display
11. Vita to be used for Introduction
12. Description of the Keynote / Seminar for Pre Publicity use
13. Masters for keynote speeches to be duplicated by hiring agency
